just got this log
Hells misses horribly! Poison wracks Yamcha's body for 2 damage. Hells has 42 health and Yamcha has 28 health.
But if I missed, surely the weapon didn't have a chance to apply the poison?

Poison damage has no correlation to physical damage. While it does leave for situations like this that doesn't make much sense if you really think about it, it's simply in the mechanics of how the system works. I will consider making adjustments to future releases to make it more logical, but don't want poison (or taint as it is also effected by this) to be tied to physical damage. Will think on it.

Then what is the 2% poison damage a percentage of? That makes no sense...

Poison does 2 things: 1) direct damage (same number as whatever percent it is) and 2) lowers your opponent's attack and defense next turn (by that much percent). Poison is actually unique among the stats in that it has a both static and percentage based effect.

perhaps you should make that clearer then in the item description.
I know poison was no correlation to physical damage, but surely it can't be hard to put in something like 'if miss then poison = 0'.

True, it would not be a difficult code change, but making that change would make accuracy & dodge more influencial (if only slightly), when I think few would argue that these stats need to be bumped up. Horray the trade offs in balancing.

In the next versions, you might consider changing poison into a modifier which has to be "triggered off" in order to be active. Your first good hit triggers off poison, and thence on you deal posion damage as it is currently calculated. It would make sense then that you can deal posion damage when not hitting, because you hit at least once before.
