Just saw this one again (previous times in locked bug thread), but under explainable circumstances:
'used' a tali, delayed a few secs while checking new item stats, then clicked enhance.
got error message, when I went back to inventory, both tali and item were still there. turns out, someone collected an 'offer' item during the delay, so the inventory items shifted before I clicked 'enhance'.
just mentioning this in case it's useful info - dunno if it's a preventable occurance, or something that will just happen from time to time...
